  2. Fill in your shipping and billing information

  3. Choose the items you want from the Music Mart product list, enter the quantity and multiply to get the amount

  4. Calculate the total

  5. Either provide your credit card info or enclose your check

  6. Mail or fax it ("where to send" info is on the form)

    7. That's all there is to it!
